Susan McGowan, 58, suffered organ failure within a fortnight of her first jab of tirzepatide - better known under its brand name Mounjaro.
Her death on September 4 is believed to be the first in the UK linked to the drug.
Her niece Jade Campbell, who was at her side when she died, said: "Susan had always carried a wee bit of extra weight but there were never any health concerns.
"She wasn't on any other medication. She was healthy." The death certificate lists multiple organ failure, septic shock and pancreatitis as the immediate cause, with "the use of prescribed tirzepatide" recorded as a contributing factor.
Susan, who had worked a nurse at University Hospital Monklands in Airdrie for more than 30 years, often chatted to colleagues about her struggle to lose weight. After researching weight-loss jabs and seeking medical advice, she got a starting dose of Mounjaro from a registered online pharmacy.
