People attending appointments and operations can now state how they identify in the forms, said to have left many bewildered.
The drive puts "wokery before welfare", observers have said.
One trust asks patients whether they are a man or woman before dental treatment. But for other services it provides 11 choices, including non-binary, intersex, and questioning/unsure.
Critics ridiculed the lists yesterday. Tory MP Miriam Cates said: "It's tempting to laugh about the wide range of genders now on offer but the NHS must ditch this mad ideology."
Nonsense
Reform UK business spokesman Rupert Lowe said: "People are sick and tired of this woke nonsense being pumped out by the NHS, all funded by the taxpayer. It claims to be a cashstrapped organisation, yet it finds the money to push this woke tripe.
"There are men, and there are women. A man cannot give birth, and a woman cannot have a penis.
"These are biological truths any politician must publicly acknowledge and repeat.
"The entire system needs fundamental reform, starting with making every single diversity-related role within the NHS redundant, a Reform UK policy millions will agree with.
