Gary, who has type 2 diabetes and epilepsy, was reportedly denied entry to a taxi outside Bar 67 and was later spotted on CCTV unconscious in a doorway.
His heartbroken family, who have travelled to the Spanish island to conduct their own search, say they have to accept the likelihood he has died.
His sister, Michelle Shearer, said: "The past seven weeks have been a living nightmare and we've had to accept and come to terms with the likelihood that Gary is no longer alive.
"We know he needs daily medication and wouldn't be able to survive this amount of time without it."
Gary's loved ones including mum Ann, 73, stepfather Danny, 65, and Michelle, 43 say they will not give up their campaign to find out what happened to him. They have vowed to return to Lanzarote to ensure the authorities are committed to the ongoing search for the dad.
Michelle added: "Our lives are in complete turmoil and we face the daily agony of having no answers.
