Kimberly Cheatle faced lawmakers' questions at a contentious House hearing in the wake of the assassination attempt against the former US president on 13 July.
In her opening statement, Cheatle acknowledged the Secret Service had "failed" when Thomas Matthew Crooks shot at Trump from a rooftop near the former president's campaign rally in Butler, Pennsylvania.
Trump sustained an injury to his ear, and one rally attendee, the former fire chief Corey Comperatore, was killed. Two others were injured.
