Mark Dickey, 40, developed serious stomach problems on 2 September when he was more than 1,000 metres (3,300ft) underground.
He was exploring the Morca cave, a remote complex of narrow tunnels in the Taurus mountains of southern Turkey.
An international effort to bring him to the surface began on Saturday afternoon. He was moved on a stretcher through tight passages dripping with water. There were stops at camps along the way, with Dickey given rest and medical treatment.
It was a perilous operation as the rescue team made its way through dark subterranean chambers, amid chilly temperatures of 4-6C (39-43F).
Turkey's caving federation said he finally emerged at 12.37am local time yesterday. Footage showed Dickey being brought to the surface, bound tightly to a rescue stretcher.
"It is amazing to be above ground again," he told reporters and friends who greeted him outside the cave. He admitted: "I was underground for far longer than ever expected."
