The Care and Support Alliance which analysed NHS England data said the major differences in provision were "startling".
The North East had the highest average rate of accepted requests for care for older and disabled people at 64% and the West Midlands had the lowest at just 32%.
The lowest rate of refused requests in a single local authority was 12%, while the highest was 85%.
A request for social care from a local authority can be rejected for various reasons, including that the person is not struggling enough or they have too much money to qualify for state-funded help.
